Failed calls and slow endpoints (root key only)

Summary of failed and slow requests for the admin monitor. Only failures (5xx, 429 and 4xx other than 401 and 404) and slow calls (at or above `REQUEST_LOG_SLOW_MS`, default 1500 ms) are stored, so this shows what is breaking and what is slow, not an overall error rate. Rows hold the method, a route with ids replaced, status, duration and request id - never query strings, bodies, tokens or IPs. When storage is not ready the response has `available` false and a note. Needs the root key.
