Combined WHO growth assessment (every applicable indicator in one call)

Runs every WHO indicator that applies to the child's age and the measurements supplied: length/height-for-age, weight-for-age, weight-for-length/height, BMI-for-age, head circumference-for-age, MUAC z-score and fixed MUAC cut-offs. Each result is identical to its own endpoint. Indicators that cannot be calculated are listed in not_assessed with the reason; a bad value in one measurement only errors that indicator. attention lists everything outside the normal range, most serious first. acute_malnutrition_signals combines weight-for-length/height and MUAC but cannot assess oedema. Screening aid only, not a diagnosis.

Default is cmam for 6-59 months (matches acute_malnutrition_signals), who_hiv_2008 otherwise.
