Classify a 6-month head circumference increment (birth to 24 months)

WHO Child Growth Standards 6-month head circumference increment (simplified field tables, -3 SD to +3 SD). This is head growth VELOCITY, not head circumference for age. Pass the start age of the 6-month window, plus either increment_cm or both hc_start_cm and hc_end_cm. Strict cut-offs: very low below -3SD, low below -2SD, within expected range from -2SD to +2SD, high above +2SD, very high above +3SD. z_approx is interpolated between the printed SD points (null beyond -3 or +3). Screening aid only, not a diagnosis.
