Create a consumer API key

POST /developer/keys

Creates a key with the `consumer` role. The raw key is returned once; only its hash is stored. Up to 3 active keys per account, and the account email must be confirmed. A consumer key gets 3x each route's rate limit (tracked per key) and FHIR scopes `system/*.*` (never AuditEvent). It can never reach admin routes or write data.
