Translate a Chakudya code (LOINC, HL7 practitioner-role, v3 ObservationInterpretation)

POST /fhir/ConceptMap/$translate · API key required

Parameters `system`, `code`, optional `url` (ConceptMap id) and `targetSystem`. Returns Parameters with `result` and one `match` per mapping. A code with no true mapping gives result=false; nothing is guessed. It is an operation, not a conformance read, so it needs an API key (scope `system/ConceptMap.read` or a wildcard).
