Analyze a meal's macro split, food groups, and (optionally) targets (API key, rate-limited)

Builds on the same ingredient resolution as /recipes/calculate but frames the result around one eaten meal: macronutrient % of calories (Atwater 4/4/9) compared against the standard adult IOM AMDR range, which core food groups are present/missing, and - only when `daily_targets` is supplied - how the meal compares to them. Never infers a personalized target itself; that should come from a clinician or the caller's own EER/macro calculation. Set `conditions` to also run clinical flags (e.g. "food_allergy" requires a matching `allergens` array).
