Pre-pregnancy BMI category and gestational weight gain

IOM totals and weekly rates by pre-pregnancy BMI (Table 14.11, Appendix 3), first-trimester allowance (1-3 kg, 0.5-2 kg if obese), twin totals (provisional; none for underweight), notes for triplets and quadruplets. With current_weight_kg and a gestational age, returns below/within/above against an approximate expected range (a straight line from the first-trimester allowance at 13 weeks to the printed total at 40 weeks). weigh_ins gives a series and the recent weekly rate. Pregnant adolescents get the Box 14.3 risk factors.
