Storage trigger webhook (shared secret)

POST /rag/webhook

Called by the database storage trigger when a file lands in the `rag-uploads` bucket. Authenticated with the `x-webhook-secret` header, not a bearer token. Files under `api/` (already have a job) and unsupported types are skipped with a 200. Folder picks the context: `clinical/`, `general/`, anything else is `both`.
